Barefoot Summers, Autumn Heel Pain

There is a pattern that podiatry clinics see every year and almost nobody warns about: heel pain that begins in early autumn.

It rarely gets connected back to its cause, because the cause was three months earlier and felt like a good time.


What a summer does to your feet

Flat sandals and flip-flops. Most summer footwear has minimal heel-to-toe drop, little arch support, and no heel counter. Many require your toes to grip slightly to keep them on, which changes how the foot works over thousands of steps.

Bare feet on hard floors. Tile, laminate, decking, poolside. These are unforgiving surfaces, and without shoes your own tissues absorb all of the impact.

A different activity pattern. More walking on holiday, often long days on hard pavements in unsuitable footwear. Or the opposite — a much less active few weeks, so tissue tolerance drops.

Then autumn arrives. Structured shoes with a different heel height. Hard indoor floors all day. The routine returns at full volume on one specific Monday.

That is a load spike in both directions, and load spikes are where this condition begins.


Why change matters more than barefoot

The barefoot argument gets fought as though it were about ideology. It mostly is not.

There is no good evidence that barefoot living either causes or cures plantar heel pain. People thrive barefoot. People thrive in supportive shoes. The failures cluster around one thing: going from one to the other quickly.

The plantar fascia adapts to what is asked of it, but connective tissue remodels on a timescale of weeks to months — far slower than your enthusiasm or your calendar. A foot that has spent a decade in cushioned, raised-heel shoes has adapted to that. Take the support away over a weekend and the structures that were being helped now do the work unaided, at full volume, with no build-up.

The useful principle is not barefoot-versus-shod. It is gradual.


The calf, which is what actually changed

Here is the mechanism most people miss, and it explains the autumn timing better than anything else.

A raised heel — even the modest drop in ordinary shoes — means your calf never has to lengthen fully. Spend years in them and the calf complex adapts short. Nothing hurts, because the shoes are hiding it.

Then you spend a summer flat and barefoot, which demands ankle dorsiflexion your calf has not had to provide. Or you spend the summer in flip-flops and then return abruptly to a different heel height. Either way, the demand changes.

This matters because in the Riddle (2003) matched case-control study, restricted ankle dorsiflexion showed the strongest association with plantar heel pain — ahead of body weight and hours spent standing.

Your shoes did not cause the tightness. They concealed it. The change simply revealed what was already there.


Test what your summer left you with

The knee-to-wall test, thirty seconds:

  1. Face a wall, one foot forward, pointing straight at it
  2. Keeping your heel flat on the floor, bend your knee to touch the wall
  3. Slide the foot back to the furthest point where your knee still reaches with the heel down
  4. Measure from the wall to your big toe

Roughly 10 cm or more is generally considered adequate; under about 5 cm suggests real restriction. Compare left to right.

If you are short, stretch both calf muscles daily — and they need different knee positions, which is the bit almost everyone skips:

  • Knee straight → gastrocnemius
  • Knee bent → soleus

Two minutes a day, sustained holds, no bouncing.


How to change seasons without paying for it

Transition over two to three weeks, not one day. Wear your autumn shoes for a couple of hours a day first, then longer. This applies in both directions — easing into sandals in June deserves the same treatment.

Do not debut new shoes on a full day. A new pair, especially with a different heel-to-toe drop, on the first Monday back is stacking a footwear change on top of a load change.

Mind the drop, not the brand. The single most disruptive variable is heel-to-toe drop. Going from flat sandals to a raised heel, or from a raised heel to flat, is what your calf notices.

Wear something indoors during the change. If you have been barefoot at home all summer, a cushioned indoor shoe for a few weeks in autumn is a cheap way to soften the transition — especially on tile and laminate.

Start the calf stretches before you need them. Late August, not October.

Build up walking volume gradually if your activity changes with the season in either direction.


If it has already started

The early signal is specific: a sharp twinge in the first few steps out of bed that eases within a minute.

That is the cheapest this problem will ever be. At that stage it is a load-management issue: reduce the avoidable volume for a fortnight, get serious about both calf stretches, look hard at what changed, and rotate your shoes.

The reason it gets ignored is that it goes away by itself every morning within a couple of minutes. Walk on it until Christmas and the recovery timeline moves from weeks to many months.

The pain that vanishes after ten minutes is not the pain going away. It is the tissue warming up.


FAQ

Are flip-flops bad for your feet? Not inherently, but they offer little support and many require slight toe-gripping. The risk is wearing them for long days of walking after months in structured shoes, rather than the sandal itself.

Should I go barefoot at home? It is fine for many people. The caution is hard floors, long durations, and abrupt changes. If you have heel pain now, a cushioned indoor shoe usually feels better.

Is minimalist footwear a mistake? Not necessarily — but the transition is where people get hurt. Weeks to months of gradual change, not a weekend.

Why does my heel hurt in September when summer felt fine? Because the tissue adapted to summer, and September changed the demand abruptly. The pain follows the change, not the season.
