Ice or Heat for Heel Pain? A Straight Answer

Here's an answer you won't get from most articles on this question: it matters much less than you've been led to believe, and the evidence for either is thin.

That's not evasion. It's the actual state of the research, and knowing it protects you from spending months on a frozen water bottle while the things that would genuinely help go undone.

Both have a use. Neither is the treatment. Here's how to think about it.


Why the standard advice is on shaky ground

Ice became standard heel-pain advice for a straightforward reason: the condition is called plantar fasciitis, the suffix means inflammation, and ice reduces inflammation. Clean logic.

Except the tissue evidence undercut the premise. When researchers examined tissue from long-standing cases — most notably Lemont, Ammirati and Usen (2003) — they found degeneration rather than inflammation: disorganised, fragmented collagen and an absence of the inflammatory cells the name implies. They proposed fasciosis instead of fasciitis.

If there's little inflammation to reduce, then the main rationale for ice as a treatment largely disappears.

Which doesn't mean ice is useless. It means we should be honest about what it's doing.


What ice actually does

Ice numbs. That's the reliable, well-established effect: cold reduces nerve conduction velocity, so the area hurts less while it's cold and for a while afterwards.

That has genuine value. Pain relief lets you sleep. It lets you get through an evening. And — not trivially — it lets you move enough to do the stretching and loading work that does change the tissue.

Use ice for: acute flare-ups, the end of a day that overloaded your foot, and when pain is interfering with sleep.

Don't expect it to: repair collagen, shorten your recovery, or change the underlying condition. There's no good evidence it does any of those.

How: 10–15 minutes, with a barrier between ice and skin. A frozen water bottle rolled under the foot is popular because it combines cold with gentle movement, and it's fine — but recognise you're getting two separate things, and it's mostly the numbing you can rely on.

⚠️ Don't use ice if you have reduced sensation in your feet — diabetic neuropathy in particular. If you can't feel the cold properly, you can't feel a cold injury developing either.


What heat actually does

Heat increases local blood flow and makes tissue feel more pliable. It's comfortable, and it tends to reduce the stiff, tight sensation.

The best-supported use isn't as a treatment at all — it's as preparation. Warm tissue stretches more comfortably than cold tissue. If your morning routine involves stretching a foot that feels like a plank, warmth beforehand makes that easier.

Use heat for: morning stiffness, before stretching, and general chronic ache where the tissue feels tight rather than angry.

Don't expect it to: repair anything either.

How: warm (not hot) water, a heat pack, or a warm shower before your morning stretches. If you struggle to get onto your feet in the morning, doing the first stretch in a warm shower is a genuinely practical trick.


The simple decision rule

Since the evidence doesn't strongly favour either, use the sensible clinical heuristic:

Sharp, angry, recently provoked → ice. You want it to hurt less right now.

Stiff, tight, chronic ache → heat. You want the tissue to move more easily.

Genuinely unsure → whichever feels better. With this little evidence separating them, your own response is reasonable data.

Some people alternate — heat before stretching, ice afterwards if the session provoked things. There's no strong trial support for contrast protocols in this condition, but it's low risk and some people find it helpful.


The thing that matters more than either

Here's the part worth taking away.

Every hour you spend deliberating between ice and heat is an hour not spent on the interventions with actual guideline support:

  • Plantar fascia-specific stretching and calf stretching — the JOSPT 2023 guideline states clinicians should use both, for short- and long-term pain reduction and improved function
  • Load management — reducing the daily volume that keeps re-provoking the tissue
  • Progressive loading — the stimulus that drives collagen remodelling, since rest alone doesn't
  • Working the calf — restricted ankle dorsiflexion was the strongest single risk factor in the Riddle (2003) case-control study
  • Soft-tissue manual therapy — recommended in the guideline for joints and soft tissue of the lower limb, on the basis of low risk and consistent likely benefits

Ice and heat are comfort measures. Comfort has value — it's not nothing to sleep through the night. But treat them as what they are, and put your real effort into the list above.

The clearest sign someone hasn't understood this condition is when ice is presented as the plan. Under the degenerative model, an intervention that only reduces sensation isn't going to change tissue that needs load to remodel.


Common questions this raises

"So the frozen water bottle is pointless?" No — it's genuinely popular for a reason. You get temporary numbing plus gentle mobilisation of the sole. Just be clear that the rolling is doing more for tissue mobility than the freezing is, and neither is a substitute for stretching and loading.

"My physio told me to ice. Are they wrong?" Almost certainly not. Icing for symptom relief is entirely reasonable advice. The question worth asking them is what the rest of the plan is — because if ice is the whole plan, that's the gap.

"What about anti-inflammatory tablets?" Same logic. They can reduce pain, which is useful. Expecting them to repair degenerated collagen asks something the mechanism doesn't support. Medication questions belong with your clinician, not an article.


The bottom line

Ice numbs. Heat loosens. Neither heals.

Use whichever helps you get through the day and sleep at night — and then spend your actual effort on stretching both the fascia and the calf, managing your load, and loading the tissue progressively.

The ice-versus-heat debate is so prominent partly because it's easy: two options, a clear question, no work involved. The interventions that change the outcome are less satisfying to argue about and take months of consistency. That's an uncomfortable trade, but it's the real one.

How long should I ice for? 10–15 minutes is standard, with a barrier between ice and skin. Longer isn't better, and carries a real risk of a cold injury.

Can I ice several times a day? You can. Just recognise you're managing symptoms repeatedly, not accelerating recovery.

Is heat dangerous if it's inflamed? Given the tissue evidence points to degeneration rather than active inflammation in long-standing cases, this concern is weaker than the traditional advice implies. Use warm, not hot, and if heat clearly makes it worse, don't use it.

What if neither does anything? That's a common and unsurprising report. Move on and focus your effort on the interventions with guideline support.
